Normal.dot is the startup template. Templates in the Startup folder are
loaded after normal.dot and are used for changing / supplementing the user
interface. For more on the different kinds of templates, tabs on the file
new dialog, and locations of templates folders see

Exactly. That's what I realised on Friday after creating the thread.
I removed the startup setting and then everything appeared only once.
Is there a workaround to keep the normal.dot as the startup template and
also not have the duplicates.

I am using Word 2003.
When I click on the "Insert AutoText" from the Header and Footer
toolbar, every option is duplicated.
I have "-PAGE-" twice, "Author, Page #, Date" twice, ....
