You are probably running with "High" security settings, which disables
macros without asking (the appearance of the Control Toolbox usually
indicates disabled macros.) You can check this by setting to "Medium", which
will give you a prompt before disabling macros.

The best way around this is to place your template in either the User
Templates or Workgoup Templates folders, and make sure that the setting to
Trust Templates and Add-ins is checked in your security options.
