Default remove items from toolbar menu

Be Really careful doing this. Use Ctrl + Alt + - (the - on the Num Pad). The
cursor will grow into a thick black larger than life - sign. Hover it over a
menu which will cause the menu to drop down, then move it over the item you
want to remove and click. You have to do this carefully as whilst the cursor
is in the mode, it will permanently delete anything you click on.
