I have tried to set the header & footer removing the different header/footer
block but the header & footer go away.

I'm not sure I understand, but let's see...

You activated "Different first page" for the header and footer? Then created a
header/footer on the first page? And when you deactivate the option, the header
and footer disappear?

That's because you defined them for the first page. When you turn off the
option, they disappear; turn the option back on and they should reappear. Then
you can create a second page (type Ctrl+Enter), copy the header/footer from the
first page into the header/footer for the second page. Delete the page break.
Turn off the option.
