If I follow what you are trying to do, I can add a Fillin field in the
header and when I create a new document based on the template the Fillin
does prompt. Are you perhaps not using a template? Fillin fields
automatically prompt only when a new document is created from a template
containing the Fillin field.
If this isn't what you are using then can you expand on what you are using
for your "auto-fill" in the header?
