Default "symbol" in the "insert" menue? Mine is gone. How get back?

Note that "Alt+Ctrl+num" is an incomplete description, and even when
you make it complete (the "num" part should be "the minus key on the
number pad", often abbreviated as "Num-") it's only the shortcut for
an em dash.
