Default Problem creating Multiple page index entries in word 2007?

I think that what he asked for was an index that lists where each
occurrence of the word "budget" appears, because he seems to be
bookmarking the first occurrence of the word "budget" and not the
first and last occurrences of the word "budget."

You need to place an Index Entry { XE } field by every occurrence you
want to index. Word will do this for you; select the word to be
indexed, press Ctrl-Alt-X; click the Mark All button to mark every
occurrence of the selected text (it's case-sensitive -- if you're
marking Smith, it won't find smith). (You don't place any bookmarks.)

but it only display the budget that appear on page 2 and not the rest?
