Default Why does my table header disconnect from the table body?

Are you saying that the table splits into two? To check this, place the
insertion point in the first row, turn off NUMLOCK and then press Alt+5 (on
the numeric keypad). If only the first row gets selected, the table has been
split (which can be caused by a page break).

If the table has not been split, check the paragraph formatting applied to
rows. Has "Keep with next" or "Page break before" been applied (Paragraph
dialog box, Line and Page Breaks tab)?
