I'm going to assume here that you are using a paragraph style for your
bullets as that is most stable - if not you can do something similar with
direct formatting:

Set up a separate style that has the same formatting as your bullet style
but no bullet and an ordinary left indent in place of the hanging one (or
whatever indents you need to accommodate the extra character). The built-in
List Continue style would be a good place to start if you are using the
built-in List bullet style.

In the item to have the letter, apply this List Continue style in place of
List Bullet (or whatever).

At the start of your paragraph, insert your bold letter, followed by a
LISTNUM field. The LISTNUM will show a bullet, picked up from the current
list format.

Alternatively, if you need to place the letter in the margin, you may find
it easier to put it into a text box and anchor the box to the bullet item.
Lock the anchor.
