The former. The trick is to get the page numbering right (for that see
http://word.mvps.org/FAQs/Formatting/Print2Pages.htm, which explains how to
double-time numbers), but it is easier to do that than to try to repeat the
note numbering (unless you restart on each page); also, I would imagine that
you would need each half of the document to flow continuously from one page
to the other.

The main problem with this, related to the page numbering, is that you can't
generate an automatic TOC from calculated page numbers. What line you take
in this regard, however, depends a lot on whether you are sending the
publisher camera-ready copy, a manuscript, or an electronic document from
which the book can be printed directly. If the first, then your task is to
get the pages to look the way they should by any means necessary. If the
second, you let the publisher (and his typesetter) worry about all those
details. If the last, then you're in a heap o' trouble!

(this is a book which has to go to a printer) or do you mean there is some
way within Word by which you can interleave text pages from two separate
files? If there is I should be glad to know how.
