Default TOC and Hyperlink

The only alternative would be to unlink the TOC (otherwise anything you do
to it will be undone the next time it's updated), bookmark the headings in
each document, and manually create hyperlinks to those specific headings
(and I'm actually not sure that's possible in Word). In any case, the
requirement to unlink the TOC would make this impracticable.
