How do I run a spell check of the comments field?

Well, when I'm in Word 2003's Print Layout view, with
comments in balloons, both the auto spell check (squiggly
lines) and F7 successfully find spelling errors in the
comments.

Which version of Word are you using? What happens when you
press F7?

If you click in a word you think is misspelled, what
language do you see in Word's status bar? Is it the
language you expect to see?

If you go to Tools/Language/Set language, is the option to
not check spelling or grammar activated?
