You convert any document into a template by saving it as a template (.dot
file). I think what you mean is that you want to convert it to a protected
form; see http://word.mvps.org/FAQs/Customizat...nTheBlanks.htm and
especially the forms tutorials by Dian Chapman that this article links to.
