Default Formatting Line Numbers

Suzanne was nice enough to ask for a sample.

When I went to open that Word document from yesterday, the automatic outline
was no longer in the document (!!!???). I tried to recreate and could not. I
tried to open the PDF and receive an message that there was an error
attempting to open and read the file.

So, I cannot produce a sample and cannot recreate. Not sure what the problem
is with this - but it is resolved and I can continue with my API
documentation.

If/when it appears again - I will forward a sample to Suzanne.
