OK - I found it.

For others click View | Toolbars | Forms

So you basically use tables with the added functionality of the Forms
Toolbar... it seems.

And it seems to work OK.

We have to create about 150 different forms for a project and put them
together in an 8.5x11 book.

1 - How can you create professional looking forms in Word?

(So that each field is in a rectangle... and looks somewhat like the
Medical Registratrion Form supplied with Word as a template.)

2 - Is Word the right tool to do this?
