There is no easy way to format part of the contents of a form field. You can
format the entire form field however you want when you set up the form.

You may want to take a look at
http://word.mvps.org/faqs/customizat...PlusHeader.htm and
http://word.mvps.org/faqs/customizat...rd97Header.htm for info on
"protecting" your headers and footers. Note that the document will still be
a protected form and you'll still have limits on editing.
