Default Cross References of type "Heading" do not refresh correctly

Thanks again Cindy. We use Word 2003 by the way. And you're right, the x-ref
does take into account new text at the beginning. But unfortunately not:
- when new text is added after or over the last character.
- or if the whole heading string is replaced with new text (this kills the
bookmark and breaks the x-ref)

All very eccentric behaviour in my humble view, and a real time waster
because it means we have no choice but to eyeball check all x-refs. Agree?

Pretty much :-) It's become especially painful since these bookmarks are no
longer visible in the document. Then one at least had a warning...
