Since a table of contents in Word would insert headings in page order,
you will have to create this (unusual) TOC yourself.

You can use a three-column table, with a new row for each new heading.
Press Ctrl+Alt+U to remove the table border. Use cross-references
(InsertReferenceCross-Reference) to insert heading text and page
numbers in the appropriate cells.

Prior to printing, select the entire document and then press F9 to
update the cross-references. Also, you may want to check the option to
"Update fields" on the Print tab of ToolsOptions.
