Sorry, it isn't possible to do that in any version before Word 2003.

You can sort of fake it as follows: Put the cursor in the last row and use
Table Split Table to make a separate one-row table for the instructions.
This will create a single paragraph mark between the tables (which you can
see if you click the ¶ button). Place the cursor to the left of that mark
and insert a continuous section break. Select the paragraph mark and format
it to a font size of 1 point. That will get the two tables as close together
as possible, so they'll look almost like one table.

If there is more of the document after the table, insert another continuous
section break to isolate the one-row table in its own section.

In the Tools Protect Document dialog, choose Forms; click the Sections
button and uncheck the section(s) that should be unprotected.
