Right-click on any Toolbar and select Customise
Under Commands, Categories, select All Commands.
Scroll down the commands list until you find FIleNewDIalog.
Drag and drop it onto a Toolbar.
Select or create a button face and add to the command (which will now show
as 'Other')
Then right-click on the button and select Default Style.
