Default Flip a whole page 180 degrees

I would be pretty sure that with a bit of manipulation of the order of the
pages to print in the File Print dialog, you can use the 2 pages per sheet
option, but starting with a portrait oriented documnet without the columns.
For example, in the Pages control of the Page Range section of the dialog,
enter 1, 3, 4, 2. Then you will get pages 1 and 3 side by site and 4 and 2
side by side and if you were to put the two printed sheets back to back, you
would have 1 and 2 behind each other and 3 and 4 behind each other.
