Default Inserting header and footer breaks

See http://word.mvps.org/FAQs/Formatting...rontMatter.htm. Note that
your cover page should be page i, the TOC ii. If you enable "Different first
page," you can leave the First Page Footer empty, thus eliminating the need
for a section break between the first and second pages.
