This can happen if you have a manual page break before the heading. Instead
of using a manual page break, format the heading (or the heading style, if
this is consistent) as "Page break before."

We are using Word 2k3 on one of our very large documents it adds many
headers in twice.

I have tried deleting and retyping the headers and then re-inserting the

TOC
with no luck.
