I have a document that has several pages that are multiple page forms
numbered as €œ1 of 4, etc€�. These pull out pages are located at the end of
the document and meant to be stand alone once removed from the original
document.

I am trying to build a table of contents that will list these pages by
title, located on the top of the first page of each multiple page form and
the page number actually being the form number located in the footer of each
form.
