Hi Ray

As far as I know, Word won't be able to do that. Word's Table of Contents
feature is designed to produce an ordinary table of contents: headings on
the left, page numbers where the reader will locate those headings on the
right. It can't pick up other material (eg the form number) in the footer.

You can, however, produce a table of contents for part of a document using
the \b switch of the TOC field. See
