Indexes and page numbering
To get a page range in an index, you set a bookmark after selecting
the entire block of text spanning those pages. Name it and Add it to
the list of bookmarks. Then (quoting from Using Word 2003, p. 724)
1. Alt+Shift+X to display the Mark Index box
2. Enter the main entry and/or subentry text you want
3. Select the Page Range option
4. Choose the bookmark you set from the Bookmark drop-down
5. Click Mark
(I know it works, because I've done it.)
The page range bookmark doesn't override any other XE-fields within
that block of text -- they'll all still go into your index.

I can get the lowest levels eg cat, dog, oak tree, pansy to have page
numbers but I cant get Animals, Plants or 'Plants and Animals' to show page
numbers. *There doesnt seem to be a format option that does and I cant figure
out any field switch that I can add that will do it.
Any help greatly appreciated
