Date Options for Forms

Not sure what sort of form you mean.
See "Formatting Word fields with switches" on fellow MVP Graham Mayor's website at http://www.gmayor.com/formatting_word_fields.htm or if it's formfields, use the vba format() function. -- Hope this helps.
