Default word 2003 to PDF - links out of place

I have a 275 page word document. I have been having big problems creating
PDFs (Acrobat 6.0, all updates). All of the links from the word document --
TOC, cross-references, etc. -- are out of place in the PDF. Too high, and too
far to the right. They don't line up with the link text at all. I have found
that if I turn on tags in acrobat converter the links are placed correctly,
however the document takes over 4 hours to convert, which is just ridiculous.
Alternately, if I change the page size to 8.5x11 (from 7x8.5), the links are
also placed correctly, however we don't want the document to be that size.

Can anyone tell me what is wrong?

I have verified that I have set the same page size in the document, the
Adobe PDF printer, Acrobat Distiller, everywhere.
