How can I turn .dot document into a form that I can easily add the
information each time i need it without have to move things around everytime
I add text. I would like to avoid using Excel if at possible.

What you are talking about is what Word calls an "online form." Check this

What most people do to fix the effect of insertions on other text is to put
the insertions, fields, into table cells. If they don't want other text to
move at all, they fix the size of the table cell so it won't adjust to the
amount of text typed in.
