In Word 2003, I have revised a document and moved the headings that I want to
use for the Table of Contents into the header on each page. When I update
the TOC, I get the error message, "ERROR! NO TABLE OF CONTENTS ENTRIES WERE
FOUND." Why are my entries for the TOC not being found?

No, they can't. By design, things in a header are not on any page so they
can't show up in a table of contents.

Look into the use of the StyleRef field. I suspect you neither need nor want
your headings in your headers.
