Default Form for work; never done

I use Adobe at home, but we don't have pro at work and I need to create a
form in Word that's editable, but I want to make it form. I'm not sure how
to incorporate fillable feilds for Name, SSN, and other personal data, but
then I want to have "check boxes" to select items. Please Help!

But note that a form created in Adobe Acrobat Pro should be fillable using
Adobe Reader, which you surely have at work.

What you are talking about is what Word calls an "online form." Check this
in help. For more about online forms, follow the links at
