Default Providing hyperlinks within a single document

I am using MS Word 2003.
I want to provide hyperlinks within a single document which will open
different topics in the same document but on different pages.
Eg. I have a document in which i have the names of all the countries
arranged in alphabetical order and after that a brief description about each
country. On the first page only country names appear and suppose if i click
canada then the corrosponding page describing canada will open up. Now if i
click on Zambia on the home page then corrosponding page describing zambia
should open up.
This is what i want to achieve. So i just want to know how can I give
hyperlinks within a single document/file.

Default Providing hyperlinks within a single document

In a document you can insert hyperlinks to headings or bookmarks within the
same document. If the places you want the hyperlinks to go to are not
formatted with a built-in heading style (Heading 1-9), you need to mark them
with bookmarks.

To insert a bookmark:
Position the insertion point in front of the text you want to bookmark (or
select the text). Then select Insert Bookmark and type a bookmark name (use
a name that makes it easy to identify the correct bookmark later - spaces are
not allowed). Click Add.

To create a hyperlink that points to a heading or bookmark:
Select the text that is going to be a hyperlink. Select Insert Hyperlink
(Ctrl+K). Below "Link to", select "Place in this document". Now the headings
and bookmarks should appear in list in the middle. Select the relevant
heading or bookmark and click OK.

You can make bookmarks visible by turning on Tools Options View tab
Bookmarks. Bookmarks to points (as described above) will appear as a gray
I-beam. Bookmarks that include text will appear as gray square brackets. If
you delete a bookmark used by a hyperlink, the hyperlink will no longer work.
