Default Hyperlinks go to wrong page when doc is converted into a PDF

I have a long (170+ pages) doc that contains many hyperlinks and an extensive
index.

When I convert the doc to a PDF, the hyperlinks miss their mark, meaning
that they link you to a point in the document near the target, but are off by
a page or two or three. The farther along in the document the target, the
farther off the mark the links is.

For example, "See more information on creating data batches on page 95,"
where the words "creating data batches" would be the hyperlink.

Clicking the X-REF to page 95 actually takes you to page 95, but clicking
the hyperlink "creating data batches" takes you to, say, page 98.

After considerable sleuthing, I'm guessing that the problem has to do with
the index markers (hidden text). The link goes to the place in the doc that
WOULD be correct if the hidden text were showing (thus making the document
several pages longer).

In the PDF file, of course the hidden text is, well, hidden.

How can I set Word or Adobe to make the links go to the right place?

I am using:
Adobe Acrobate Professional v 7.
Word 2003 SP2
Windows XP SP2

Any help would be appreciated.

I assume that you have resolved all tracked changes and HIDDEN all of the
hidden text BEFORE you created the PDF?

That's essential: you are correct, Word paginates according to the text that
is visible at the time.

Yes, I did hide all the hidden text before creating the PDF and updated all
X-refs (F9) before creating the PDF.

Do you have any ideas for me?

Yes. Make a copy of the document and REMOVE the Hidden text.

Obviously the PDFMaker application is revealing the hidden text before
repaginating the document.
