Default non alphabetical index

Is it possible to arrange index entries other than alphabetically? e.g. in
the Bible Acts comes after John but alphabetically in an index, it would be
the reverse which would be confusing. Is it also possible to arrange entries
so that numbers can be recognised without integer zeros i.e so 6 comes before
61 without having to enter 06?

If you are saying that you want the entries in page order, then what you
are describing is a table of contents, possibly one generated from TC
fields. See http://sbarnhill.mvps.org/WordFAQs/TOCTips.htm.
