Default Footnote Reference missing from Footnotes

Hello,

A document has been submitted to me. There are footnotes within the doc.
Part way through, the footnote reference is missing from the actual footnote
(in the footnote section - not in the text.) All footnote references are
intact in text.

Example: There is a footnote 64 in text that does link to a footnote.
Instead of the Footnote Refence in footnotes being "64" and the Footnote
itself saying Id at 1234, the Footnote Reference is gone from the footnotes
and the "64" is actually part of the footnote itself. Things still link back
and forth between text and footnotes.

Does anyone know how to get the footnote references back into the footnotes?
