Default How do I format a box so it checks by clicking on it?

I am trying to format a form where I want users to go into an empty box,
click on it and it will change to a box with an X.

One way is to use a checkbox form field in a protected form
(http://www.computorcompanion.com/LPMArticle.asp?ID=22).

Another way, which uses macros instead of form fields, is shown at
