Default File not found: DVAZddin.dll when i open any word file. ??

everytime i try to open or close a word file i get a pop up:
File not foundVAZddin.dll
the box says it is Microsoft Visual Basic.

i have tried reinstalling Microsoft Office and it doesn't eliminate this.
would appreciate any answers/suggestions of how to fix this.

In Word, click on Tools | Options | File Locations, and note the location of
your Word startup folder. Close Word. Start My Computer or Windows
Explorer, and browse to your Word startup folder. Delete the
DVZ(something).dot file from the startup folder.

thanks, i tried that and the pop up still appears.

At some time you have installed an add-in (Dataviz?) to Word and then not
removed it correctly. See

Part of the add-in is still present and is attempting to call the DLL that
runs the add-in functions (which is not present).
