I have this problem where i cant include chapters into the table of contents
when 'Index and Tables' is used. Whenever i click on 'Index and Tables', the
chapters of the document do not appear on to the table of content. The
styling of the chapter titles is 'CHAPTERS'.

In the dialog, click on the Options button. This will allow you to assign
custom styles to TOC levels.
