add to a toolbar or the QAT.

Otherwise, you'd need to create a textbox whose layout is formatted as
anything other than 'in-line' or 'behind' the text and laid out so that
its position remains fixed on the page (ie not 'move with text') and do
this for each page.

The advantage of the first approach is that the link button is always
visible, regardless of where you are on the page and the page zoom factor.
