Default Changing the format for an existing index

I followed all the instructions on how to change an existing index & got the
error message, "Error! No index entries found". Two questions, did I miss
something & secondly, can someone walk me through on how to change the
format of an existing index?

FYI, I am using the Manual wizard provided in the Templates dialog box &
working in Word 2003.
