I cannot always replace the placeholder text with customized text. When I
save the document, some text reverts back to the placeholder text. Any
suggestions as to why this is happening and what can be done about it?

You inserted a type of content control; to delete it, just click the
placeholder text, right click and click Remove
Content Control on the shortcut menu.

The content control is using a bound field from the document's properties.
IOW, what you type in the content control will automatically update the File
Properties for the document and if you change the data in the File
Properties the change will automatically be reflected in the content
control. The reason the data is disappearing when you save is because a
security option that removes personal information when you save your
document is enabled. To rectify the issue, click the Office Button, click
Word Options, click Trust Center, then the Trust Center Settings button. In
the Privacy Options section deselect the "Remove personal information from
file properties on save".
