Default creating an index

I want to insert a word in the index such that the word on the index page
will be in lower case letters and both lower and upper case letters will be
marked in the text

If you're using a concordance, you'll need to include in one column all
versions of the Word as they appear, with the lowercase version in the other
column. If you're tagging index entries manually (which is preferable), you
can search for the word without regard to case and paste in an XE field with
the lowercase entry.
