I'm trying to create a form template in Word. Does anyone out there know how
this works?

Unfortunately, I don't. I really shouldn't be answering this note.
I am only doing so to suggest that you access Word Help.

Press F1 and enter "create a form template" in the search box.
You'll get numerous "hits". Play around with some of the sample
forms by walking through the wizards. No harm done. Examine
what's happening.

You might also be well served to browse/purchase one or more
good Word books. Some are large, intimidating, and expensive.

Others are sufficient, offer color, and are simplified. These don't
generally cover as much detail but they're great for getting
started and for learning concepts. I like one new book titled
Word 2003 Quicksteps for $17.00. It's in color and is an
easy read.

I expect that you are talking about what Word calls an "online form." You
use the Forms toolbar to create these and can enable the form fields by
clicking on the lock button on that toolbar. Check this in help. For more

Chapman's series of articles. You may also want to look at

Your form should be saved as a template and used as the basis for creating
new documents (that will have your form fields and layout).
