Default Set up a blank template letter with fill in blanks

We type up letters of appreciation and just change a few general items, i.e.
who the letter is addressed to, the location where the help occurred,
people's names, type of help. Is there a way to format a letter to show the
blanks for filling in these items. By the way, it is usually 1 letter at a
time, but different people will use the same template letter off of a share
drive

What you are talking about is what Word calls an "online form." Check this

Although you could use form fields for this as Charles suggests, another
technique commonly used in the templates that ship with Word (and very easy
to create) is MacroButton fields. See
