Default Outline with Page Headers

How can I create a page header for an outlined document?
When I create a page header, it only appears on the first page.
When headers/footers viewed on subsequent pages, they are blank.
There don't appear to be sections.
This document was created by somebody else, starting with a standard template.

Never mind. I found out the problem.
Page layout had it set to different page header for 1st page
