Default How do I create my own template document (e.g., a referral form)

I would like to create a template for a referral form. (so others can fill in
the blanks electronically). How do I do this on Microsoft word?

I expect that what you are asking about is not a Word template
(http://addbalance.com/usersguide/templates.htm) but rather what Word calls
an "online form." Check this in help. For more about online forms, follow
the links at http://addbalance.com/word/wordwebresources.htm#Forms or
http://word.mvps.org/FAQs/Customizat...nTheBlanks.htm especially Dian
Chapman's series of articles.

Hope this helps,
