Default Add (to dictionary) is grayed out.

I want to add to the Word dictionary and used to be able to. Now I find
that the Add item on the menu is grayed out. What to do?

The usual reason for this is that the text in your Word document is formatted
as a different language than your dictionary. Open the Word document, press
Ctrl+A to select all of the text, click on Tools | Language | Set Language |
choose your language | uncheck the "Detect language automatically" and "Do
not check spelling or grammar" boxes | Default | OK. Then try to add a word
to the dictionary again. See http://support.microsoft.com/?kbid=211188 "Add
Button Unavailable in the Spelling Dialog Box" for some other ideas.
