Default Captions for breaking tables

Is there any way to replicate a table caption on each page, if the table
automatically breaks across 2 or more pages? (For example, "Table 9:
Depreciation Schedule, Page 1 of 4")

Also, if this can be done in Word, can the captions be placed either above
or below each section of the table?

Place the caption in the first row of the table (Merged all of its cells and
remove left/right/top borders).

Include that row in the repeating header row(s).
