Default how to stop text from shifting when filling out a form

I am trying to make a document for my work that in essence is a form for
collecting data. As I talk to people on the phone, I fill in their address,
phone number, birthdate, etc. The form I have created has all the heading
(Name, address, phone number, etc.) laid out with spaces to type in the data.

The problem is when I type in the data, all the text to the right shifts to
the right and then moves to the next line below. Isn't there a way that I can
fill in the answers without the text to the right moving to the right (as
long as I don't type in more than the allotted space for the answer). My
guess is that the solution is simple. I just don't know what it is.

Generally, forms makers use tables with fixed column width and row height.
