Default Double counting in TOC

I prepared the document with certain style and then inserted the table of contents. One of the item/heading always appears twice. I could remove one of them by manual delete, but this would not be a long term solution because once I update the field the second heading always appears again. I have tried everything to solve this--including deleting a range of text including the heading field and then put them back in.

I suspect this is a bug of some sort or anybody has a solution?
